For older adults, caregivers, and loved ones alike, these moments can feel overwhelming.

Think of the Township as a welcoming first stop. You do not need to know the name of a program, understand the paperwork, or have all the right questions before reaching out. Our staff can help you make sense of the options and identify the next steps that fit your situation.

A major part of that support is benefits and Medicaid counseling. Our benefits specialists can help residents and families understand eligibility, review available assistance, and navigate applications for programs such as Medicaid, Medicare, SNAP, prescription assistance, Ride Free Transit, and other state and federal benefits.

If Medicaid is part of the conversation, staff can help explain the process in plain language, assist with applications, and connect you with other resources when additional support is needed.

Our team can also help with reviewing your options, housing and financial resources, caregiver support, and referrals to other Township and community programs.

In addition, a Care Coordinator can visit the home to conduct a comprehensive assessment of need and develop and individualized care plan including appropriate connection and linkage to services.

You do not have to figure everything out on your own. And you do not have to know exactly what to ask for before you call.
